What does the "light map" of the face mean?

The "light map" is the individual distribution of light, shadow, projection and transitions across the face. In Fillart it serves as an aesthetic planning principle, so that treatment does not follow individual lines or standardised points alone. Face shape is perceived strongly through how light falls on the cheeks, midface, the lid-cheek transition, the chin and the jawline; even small changes in volume or contour can affect that perception. The face is therefore examined from several perspectives: frontally, from the side and in the three-quarter view. Natural asymmetries and the effect in movement can also feed into the planning.

Why does Fillart work with micro-boluses?

Fillart or a Skin Booster – what is the difference?

Skin boosters aim primarily at skin quality and hydration. Fillart aims more at targeted filler placement, contour and the spatial balance of the face. At AS Clinic, Fillart is planned more freely, according to the individual distribution of light and shadow and the contour the face actually needs. That means the treatment does not have to follow a fixed number of points: fewer or more small placements can make sense, as long as they are anatomically justified and coordinated with each other.

Fillart or Botox – what is the difference?

Fillers change volume, projection and contour. Botulinum toxin reduces muscle activity in a targeted way. The two therefore treat different causes. An expression line caused mainly by muscle movement is not automatically a filler problem; conversely, botulinum toxin cannot fill a structural volume deficit. Before treatment it should therefore first be established whether muscle activity, volume, skin quality or a combination causes the change you can see.

When do you see the result?

With volumising fillers, a first change in contour is visible immediately after treatment. The final result should not be assessed straight after the injection, however, because swelling and the tissue reaction can temporarily change the proportions. In the first few days, treated areas can look somewhat fuller or less even; only once the swelling subsides can you assess how harmoniously the small filler deposits fit into the overall picture.

How long does Fillart last?

There is no blanket longevity figure for Fillart, because the technique is not tied to a single product. The duration depends on the filler used, the region treated, the amount, the injection plane and individual factors. More mobile regions can behave differently from areas treated structurally. A top-up should therefore not follow a fixed calendar automatically: before every repeat treatment it should be checked how much product and volume is still present and whether a further injection is genuinely necessary.

Can Fillart be dissolved again?

Whether a filler can be dissolved depends on the material used. If a hyaluronic acid filler is used, it can generally be broken down with hyaluronidase; that does not automatically apply to other filler materials. Dissolving is itself a medical treatment and is only carried out after an individual assessment.

Does Fillart hurt?

How much you feel is individual. At AS Clinic, local anaesthesia is used for Fillart. During the micro-injections you may still feel brief pinpricks, pressure or a sensation of tightness. Sensitivity can differ depending on the facial region treated.

How long is the downtime?

Downtime is usually minimal. Small swellings, redness or bruising can occur, however, and remain visible for a while. If a wedding, photoshoot, trip or important work appointment is planned immediately afterwards, allow an appropriate gap.

Risks and possible side effects

Fillart is also an invasive filler treatment and not free of risk. The more common reactions are temporary swelling, redness, tenderness and small bruises. A rare but serious complication of filler injections is vascular occlusion, in which the blood supply to an area of tissue can be impaired; the individual risk depends among other things on the region treated and the injection technique. Possible reactions are:

• Swelling and redness
• Tenderness or pain
• Small bruises
• Temporary irregularities or asymmetries
• Palpable nodules
• Inflammatory reactions
• Infections
• Further reactions depending on the product and area

Which warning signs need immediate assessment?

Severe or increasingly unusual pain, striking blanching, mottled grey or bluish discolouration of the skin, and sudden visual disturbances should be assessed medically at once. Sudden visual disturbances or neurological symptoms after a filler injection are a medical emergency and should not be left until a regular review appointment.

Who may Fillart be suitable for?

Fillart may be an option for adults who want a subtle, targeted filler treatment and where several small contour or volume deficits can sensibly be planned together. Whether the treatment is genuinely suitable can only be assessed after a personal analysis. Possible starting points are:

• A wish for a conservative treatment that does not emphasise volume across the board
• Small structural volume deficits
• Uneven transitions between adjacent facial regions
• Slight natural asymmetries

When may Fillart not be the right treatment?

Not every visible change in the face is caused by missing volume. Where muscle activity, skin quality, fatty tissue, pronounced skin laxity or another structural problem is the main factor, a different treatment concept can make more sense. Depending on your state of health, the product used and the area treated, treatment can also be postponed or excluded — for example with acute infections or inflammation in the treatment area, pregnancy or breastfeeding, relevant allergies, certain clotting problems or other medical risk factors. Prescribed medicines should never be stopped or paused on your own initiative for an aesthetic treatment.

What should you bear in mind after Fillart?

You will receive individual instructions after treatment. In general it can make sense to:

• Touch the treated areas as little as possible at first
• Avoid unnecessary firm pressure on the treated areas
• Not massage on your own initiative, unless this has been expressly recommended
• Keep the injection sites clean
• Avoid intense physical exertion at first
• Avoid strong heat and saunas for a while
• Have unusual or increasingly severe symptoms checked early
